About This Position
Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) specializing in Long Term Care to join our healthcare team. The ideal candidate will possess a strong foundation in patient care, with expertise in managing the unique needs of residents in long-term, assisted living, and memory care settings. As an LPN in this role, you will be responsible for delivering comprehensive nursing services, supporting patient well-being, and ensuring compliance with healthcare standards. Duties
Provide direct patient care, including medication administration, wound care, and vital signs monitoring in accordance with physician orders and facility protocols.
Assist with activities of daily living such as bathing, dressing, mobility support, and feeding to promote independence and comfort.
Conduct patient assessments and document medical information accurately within electronic health record (EHR) systems like Epic or Cerner.
Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to residents' needs.
Support infection control practices and ensure adherence to HIPAA regulations to maintain patient confidentiality and safety.
Administer IV infusions, injections, and perform catheter care while maintaining aseptic techniques.
Educate residents and their families on health management strategies, including dementia care, Alzheimer's support, and pain management techniques.
Participate in discharge planning, case management activities, and emergency response procedures such as triage or airway management when necessary.
Requirements
Valid licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in the state of practice.
Proven experience working in long-term care facilities, nursing homes, assisted living communities, or similar settings.
Familiarity with EMR/EHR systems such as Epic, Cerner, or Athenahealth for medical documentation and patient records management.
Knowledge of ICD-9/ICD-10 coding for accurate documentation and billing purposes.
Experience with specialized care areas including dementia care, memory support units, hospice care, or acute pain management is highly desirable.
Ability to perform physical examinations, vital sign assessments, venipuncture, phlebotomy, and sterile procedures such as suturing or spinal taps when required.
Strong understanding of physiology, anatomy, pharmacology related to medication administration, and infection control protocols.
Excellent communication skills for effective patient assessment and health coaching; ability to work collaboratively within multidisciplinary teams.
